<p>This 12-month post-graduate program provides clinical and academic experience for new graduates transitioning into professional roles as PMHNPs.  The San Francisco VA Healthcare System, with academic partnership from UCSF, promotes mentorship and fosters a supportive interprofessional learning environment for providing evidence-based care to our Veteran population. </p>
<h2 id="program-structure"><strong>Program Structure</strong></h2>
<ul>
<li>This is a 12 month program</li>
<li>Residency curriculum includes:
<ul>
<li>Clinical rotations on interprofessional teams</li>
<li> Integrated and parallel didactic sessions</li>
<li>Learner-selected quality-improvement project</li>
<li>Didactic sessions frequently occur in conjunction with other UCSF-SFVAHCS resident training programs to promote interprofessional learning</li>
<li>Core clinical rotations include longitudinal learning experiences in general outpatient psychiatry, addiction and recovery treatment, PTSD, and women&#8217;s health</li>
<li>Block clinical rotations include the psychiatric emergency service, community mental health, TMS, and psychotherapy groups</li>
<li>Residents are assigned a faculty mentor and advanced practice nurse preceptors during clinical care rotations to serve as resources and provide support as they transition to performing as autonomous PMHNPs</li>
</ul>
</li>
<li>Upon successful completion of the residency a certificate is awarded</li>
<li>Candidates who successfully complete the residency training program will be given the</li>
</ul>
<p>opportunity to apply for available vacancies at the SFVAHCS</p>
<h2 id="applicant-qualifications">Applicant Qualifications</h2>
<ul>
<li>Must be a US citizen</li>
<li>Must have graduated from an advanced practice psychiatric nursing program within the 12 months preceding the residency start date</li>
<li><strong>The residency must be your first position serving as an NP in the same specialty as the program&#8217;s focus</strong></li>
<li><strong>ANCC PMHNP board exam completion within 90 days of employment</strong></li>
<li>Licensure: Current, full, active, and unrestricted registration as a Registered Nurse in a State, Territory or Commonwealth (ie, Puerto Rico) of the United States, or the District</li>
</ul>

<p>The Post Baccalaureate RN Residency Program (PB-RNR) is a nationally funded training program through the Office of Academic Affiliations (OAA).  The mission of the San Francisco VA PB-RNR is to provide a one-year transition to practice training program for post baccalaureate registered nurses supporting role socialization through robust, veteran-centric clinical and didactic learning experiences.  The PB-RNR offers new nurses the opportunity to gain confidence and competence through enhancing the knowledge, skills, and attitudes necessary to care for our nation&#8217;s veterans.</p>
<h3 id="about-the-pb-rnr"><strong>About the PB RNR</strong></h3>
<p>Throughout the one-year PB-RNR program, the resident nurse is supported with 100% protected learning time to master the complexities of providing nursing care as an advanced beginner to a competent care provider.  The robust veteran centric curriculum is designed using the 2021 CCNE Standards for Accreditation of Entry-to-Practice Nurse Residency Programs and consists of a comprehensive four-part clinical rotation and didactic training program.  Residents participate in precepted clinical rotations in ambulatory, acute and specialty care areas and conclude the program with leadership development training.  Nurse residents participate in engaging and interactive didactic learning experiences, specialized training opportunities, interprofessional collaboration, and professional role socialization in areas like evidence-based practice, quality improvement, and early career development.  Upon completion of the one-year PB-RNR, nurse residents are eligible for hire in a variety of locations within the SFVAMC.</p>
<h4>PB-RNR Program Objectives</h4>
<p><strong>At the conclusion of the Post-Baccalaureate Registered Nurse Residency Program the participant will be able to: </strong></p>
<ul>
<li>Demonstrate enhanced confidence and competence from entry-level, advanced beginner nurse to competent, autonomous professional nurse through a one-year, veteran-centric PB-RNR.</li>
<li>Apply the knowledge, skills, and attitudes necessary to provide effective decision-making skills and sound clinical judgment for the care of veterans.</li>
<li>Use effective communication and collaboration skills with nursing and interprofessional teams to ensure quality and safety in the care of veterans.</li>
<li>Apply research and develop evidence-based practice standards to support quality and safety in the care of veterans.</li>
<li>Demonstrate professional role socialization as a member of the health care team through the one-year PB-RNR.</li>
<li>Apply clinical leadership skills in the care of veterans.</li>
<li>Create an individual nursing career plan promoting lifelong learning and a commitment to professional nursing practice.</li>
</ul>
<h3 id="nurse-resident-qualifications"><strong>Nurse Resident Qualifications</strong></h3>
<ul>
<li>Be a graduate of baccalaureate or master&#8217;s level entry to practice program with a 3.0 or higher from an accredited school of nursing by the Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing (ACEN).</li>
<li>Possess a current, full, active, and unrestricted registration as a graduate professional nurse in a state, commonwealth or territory of the United States, or the District of Columbia, prior to the start of the PB-RNR program.</li>
<li>Be a citizen of the United States.</li>
<li>Be serving in his or her first Registered Nurse role.</li>
<li>Be a recent graduate within the past 12 months.</li>
<li>Provide proof of current immunizations.</li>
<li>Pass the VA pre-employment medical exam/drug testing</li>
</ul>
<h4><strong>Upon Selection to the PB-RNR the resident will receive:</strong></h4>
<ul>
<li>12-month grant in the amount of $68,174</li>
<li>Health insurance</li>
<li>Accrued annual and sick paid leave</li>

<h2 id="training">training</h2>
<p>NP residents work as members of an interdisciplinary team in collaboration with internal medicine, pharmacy and psychology residents, social workers, dieticians and other allied health professionals.  Teams collaborate to provide excellent care for Veterans using a patient centered approach.  Primary care clinics are mentored by NP and MD faculty to facilitate the development of skills in patient-centered and team-based care and communication, clinical reasoning and diagnostic skills, continuous quality improvement, and equity-based and scholarship activities.  Specialty placements are a requirement of the program with the option of either longitudinal or rotation-based experiences.  A variety of specialty placements are available, and every effort will be made to match the resident with the specialty experience of their choosing.</p>
<h3 id="primary-care"><strong>primary care</strong></h3>
<p>Nurse practitioner residents are responsible for a panel of approximately 200 primary care patients with supervision.  NP residents work in a patient-centered medical home model (called PACT: Patient-Aligned Care Team within the VA) with a “teamlet” consisting of resident physician practice partners, a registered nurse case manager, licensed vocational nurse, and clerical staff.  A pharmacy resident, behavioral health psychology fellow, social work intern or dietetics fellow may also be included in their clinical care team.  NP residents conduct routine follow-up, new patients, telephone and video-based care, women&#8217;s health, and urgent care visits.</p>
<h3 id="rotations"><strong>rotary</strong></h3>
<p>Specialty placements are a requirement of the program with the option of either longitudinal or rotation-based experiences.  Options may be available in cardiology, dermatology, pulmonology, palliative care, gastroenterology, liver, long-term care, pain, hematology/oncology, emergency department, care for the unhoused, wound care, nephrology, endocrinology, infectious disease.</p>

<h4>The PGY1 pharmacy residence (NMS code 188313)</h4>
<p>The PGY1 Pharmacy Residency offers a flexible program that offers a focused clinical, teaching, and administrative experience with experienced, individualized guidance.  Required clinical rotations include acute care (internal medicine, critical care, or surgical specialty) and ambulatory care.  The resident will also gain experience in other required learning experiences such as:  B.: Introduction to drug safety;  inpatient surgeries;  Forms management, teaching certificate and primary residence/pharmacy management.  Elective rotations are available for infectious diseases;  inpatient anticoagulation;  pain therapy;  Oncology, Advanced Internal Medicine, Drug Safety, Pharmacy Administration and Geriatrics/Long Term Care.  Residents may prescribe UCSF and UOP IPPE and APPE students.  Other responsibilities include: attending emergency medical care (code blue and acute stroke);  and Completion of a quality improvement or research project presented at the Western States Conference for Pharmacy Residents, Fellows and Preceptors.  All PGY1 residents actively participate in the EdPACT (Education in Patient Aligned Care Team) program (EdPACT), which promotes a patient-centered, interprofessional team environment.</p>
<h4>The PGY1 pharmacy residence with geriatric electives (NMS code 188322)</h4>
<p>One position is reserved each year for residents who desire more experience in geriatrics.  Residents who choose geriatric electives complete the same core rotations as other PGY1 pharmacy residents in the San Francisco VA Health Care System, but also complete 12-week geriatric rotations in place of other electives.  Applicants must apply separately for this program using the NMS code listed above.</p>

<h2 id="pgy2-pain-management--palliati">PGY2 Pain Management and Palliative Care Residency Program</h2>
<h4><strong>program purpose</strong></h4>
<p>Postgraduate year two (PGY2) pharmacy residency programs build upon the Doctor of Pharmacy (Pharm.D.) training and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice.  PGY2 residencies offer residents the opportunity to function independently as practitioners, conceptualizing and integrating accumulated experience and knowledge, and incorporating both into the delivery of patient care or other advanced practice environments.  Residents who successfully complete an accredited PGY2 pharmacy residency will be prepared for advanced patient care, academic, or other specialized positions, along with board certification when available.</p>

<p>The PGY2 Pain Management and Palliative Care Pharmacy Residency at the San Francisco VA Health Care System is a year-long residency designed to prepare a pharmacist to practice as a Clinical Pharmacy Specialist in pain management and/or palliative care.  The program is designed in accordance with the PGY2 Pain Management &#038; Palliative Care Pharmacy Residencies competency areas, goals and objectives required by the American Society of Health-System Pharmacist (ASHP).  Direct patient care experiences, case-based application, and choice experiences are designed to engage the resident in a variety of content areas: acute pain management, chronic pain management, psychiatric conditions, palliative care, hospice, interventional medicine, and integrative medicine.</p>

<p>A program to alleviate teacher shortage in San Francisco has lost a key partner after fraud allegations that resulted in a $ 2.5 million settlement.</p>
<p>The University of San Francisco pulled out of the San Francisco Residency Program, launched in 2010, last month to help educators in education settle into classrooms during a teacher shortage.</p>
<p>Participants spent five hours a day in a classroom in the San Francisco Unified School District for the year, earning a scholarship, and getting used to the system as they pursued their teaching credentials.  The program was announced to attract and retain educators with cultural literacy to mentor students in San Francisco, an ongoing problem in a high cost of living city.</p>
<p>But in March, USF announced internally that it would part ways with the program, the auditor learned.</p>
<p>The change comes after USF agreed to pay the federal government $ 2.5 million last May in a settlement with the U.S. Attorney General on fraud allegations.  Now the university is at odds with SFUSD over how much of this settlement the district itself might have to pay.</p>
<p>USF called his departure a &#8220;difficult decision&#8221; and cited disruptions from the pandemic.</p>
<p>&#8220;Looking ahead, we are taking the time to decide if and how we would redesign the residency program with SFUSD,&#8221; USF spokeswoman Kellie Samson said in an email to the auditor.</p>
<p>The allegations of fraud surfaced when a whistleblower alleged that then program director Jonathan Osler falsified timesheets between 2014 and 2016 to get funds from AmeriCorps grants.</p>
<p>In a report to Congress released in September, the Federal Inspectorate found that USF had forged more than 1,500 timesheets and certified 61 awards during that period through its director, who received the grant.  The Timesheets qualified USF to receive more than $ 1.7 million in grants through the Corporation for National and Community Service known as AmeriCorps.</p>
<p>According to the recommendation of the CNCS-OIG, AmeriCorps excluded Osler from the federal government on January 27th.  This means that Osler can neither apply for or receive federal contracts nor act as a representative for other contractors until February 2024.  Any affiliation with an organization that does business with the government must be &#8220;carefully considered&#8221;.</p>
<p>Osler is currently the Senior Director of Schools, Programs and Learning for the Oakland Public Education Fund, which works with the Oakland Unified School District.  He joined the nonprofit as development director in May 2018 and rose to his senior position in October, according to the fund&#8217;s website and Osler&#8217;s LinkedIn.</p>
<p>&#8220;I firmly believe in what we wanted to achieve at SFTR: ensure that students from underserved schools have access to well-prepared teachers from communities like yours,&#8221; Osler said in an email.  &#8220;I also believe in the collaboration we have had and hope that it can be repeated in other locations in the future.&#8221;</p>
<p>SFUSD spokeswoman Laura Dudnick said the district has reviewed its systems and is now allowing residents to receive a cost of living scholarship by participating in the program rather than based on specific activities recorded on a timesheet.</p>
<p>&#8220;One of the whistleblower&#8217;s complaints related to irregularities in time reporting for the AmeriCorps Grant, which was issued to SFTR partner USF,&#8221; Dudnick said in an email.  “The timing irregularities related to a practice allegedly run by an employee overseeing the SFTR program.  This person is no longer an employee of the district and has no participation in the SFTR program.  &#8221; </p>
<p>USF asked SFUSD to pay part of the settlement.  It&#8217;s unclear how much USF is looking, but the demand comes at a time when the urban school district faces a projected structural deficit of $ 112 million by next year. </p>
<p>&#8220;The university has asked SFUSD that SFUSD pay its fair share of the settlement,&#8221; Samson said in an email last month.  “SFUSD previously agreed to pay its fair share.  SFUSD has not yet fulfilled its obligation.  The matter is in formal dispute resolution.  &#8220;</p>
<p>&#8220;The district was not a party to the dispute that led to the settlement,&#8221; said Dudnick.  “This litigation was against USF, the Americorps scholarship holder.  The district has not agreed to contribute to the settlement and does not currently expect to contribute.  &#8220;</p>
<p>When the residency program was launched, the city made an effort to attract and retain educators for the urban school district.  A 2015 report showed that 80 percent of program graduates remained in SFUSD after five years, compared to 50 percent of fellow SFUSD teachers and 20 percent of Teach for America graduates in the district.</p>
<p>The problems of the program and the loss of its retention rate benefits come at a time when SFUSD is already grappling with the structural deficit, insecure enrollment due to the pandemic, and ongoing work to get all students to a regular by the fall semester Bring back schedule.  Layoffs are not expected because the district failed to issue notices by the required March deadline.</p>
<p>The program, now referred to as the Teaching Residency Program in SFUSD, has 23 residents from Stanford University and the USF this year.  Dudnick said the program expects six to 12 residents from Stanford next year, plus six to 15 residents from other residency programs at San Francisco State University / Trellis and New York University. </p>
<p>United Educators of San Francisco President Susan Solomon, who served on the residency program advisory board, found the timesheet fraud &#8220;disappointing&#8221; and &#8220;surprising&#8221;.  However, she hopes the district can stick to the program.</p>
<p>&#8220;Residency programs are a model program to really prepare people to become teachers,&#8221; said Solomon.  “These are people who really learn, not only do excellent exercises, but get to know the school district really well and then commit to staying.  It&#8217;s an excellent program that we fully support.  &#8220;</p>
<p>When asked about rebuilding confidence in the future program, current director Jennifer Steiner said the deal came a long time ago and she feels that the program has since made headway.</p>
<p>&#8220;I don&#8217;t know that there is necessarily a trust problem on the table,&#8221; said Steiner.  “We will maintain the integrity of everything that was good about the program.  If USF decides to come back to the table, they are welcome.  &#8220;</p>
