<p>” Oct. 8.) Yet, mounting evidence shows that housing-first policies fail to address the underlying drivers of chronic homelessness — mental illness and substance abuse. It’s something policymakers are increasingly coming to understand. Similarly, as the public has witnessed and endured the consequences of these quixotic policies, opinion has shifted away from policies that deprioritize and, in many cases, outright ignore the crucial role of mental health and substance abuse treatment in favor of policies that center on accountability, transparency, and long-term outcomes. Housing-first policies do little to help program participants transition to independent and flourishing lives. To make matters worse, the qualifications for housing disincentivize mental health stability, sobriety, and financial well-being. And the housing-first approach isn’t just ineffective — it’s expensive. San Francisco has a $1.1 billion budget that is mainly allotted to permanent housing, in addition to federal funding that flows into the city for the same purpose. Like a line from the famous Eagle’s song, results have been disastrous and even deadly — trapping people in their own devices. An investigation by the San Francisco Chronicle found that despite the program’s high price tag, individuals were living in squalor, with toxic mold, dilapidated ceilings, and rampant crime. Sadly, 166 people overdosed in these hotels between 2020 and 2021. Housing operators who turned a blind eye to dangerous drug use and crime in their housing units never faced any accountability for these failures. Though program proponents claim that “wrap-around services” are a central tenant of their policies, all participation in those services is optional. And there is little transparency into how widely available those services really are. Studies have found that Housing First programs do nothing to reduce criminal activity among homeless people. These cracks in the system have stretched across the country. St. Louis is not immune. Making services and treatment optional for tenants struggling with substance abuse and mental illness is woefully ineffective and carries fatal consequences. A 2021 study by the Urban Institute found that 12% of Denver permanent supportive housing program participants died over three years, compared to 8% in the control group who did not enter the program. The good news is that there is a better way for cities like St. Louis to address this growing crisis. Empowering people to transition out of homelessness through short-term shelters and mental health and substance abuse treatment would help them get a stable footing while building a community of support. A meta-analysis of studies comparing the impact of housing-first programs and recovery programs on substance abuse found that the latter had much more effective outcomes for those looking to rebuild their lives. But accountability and recovery can’t stop there. Those who refuse to take advantage of much-needed services like short-term shelter and treatment services for mental health and substance abuse cannot be allowed to put themselves and others in the community in danger. A vital component of this is removing and cleaning up dangerous tent encampments, as Phoenix has recently done. To do this, policymakers must swiftly address the shortage of temporary shelter space. The creation of sanctioned camping facilities away from residential and business areas is one immediate solution. Sanctioned camps can be monitored by law enforcement and provide sanitation, clean water, and services, creating a safer environment for homeless individuals than city streets. Denver recently took this approach, serving 242 people in 2021 and helping 47 to transition into more stable housing. Despite crime in Denver increasing by 14% during this time, crime in the sanctioned camp actually decreased. Policymakers should consider ways to expand short-term shelter options as well. St. Louis has options and examples of alternatives to housing first that work well in addressing the homelessness crisis. Providing immediate help to individuals on the street is a more compassionate path to sustained independence — and growing research shows the public supports it. Pursuing innovative policies that target the underlying causes of homelessness will help restore communities and the lives of those experiencing homelessness.</p>

<p><strong>TEAMS COMPARISON:</strong> Sunday&#8217;s game features two top ten special teams units built with different philosophies. The Burrow Bengals have stitched it together with largely first and second-year players, such as rookie punter Brad Robbins, rookie personal protector Jordan Battle, rookie punt returner Charlie Jones, rookie gunner Andrei Iosivas and second-year long snapper Cal Adomitis. Although the two linchpins are vets, linebackers Akeem Davis-Gaither and Markus Bailey, Davis-Gaither just returned from a month-long knee injury. In his first year playing, safety Tycen Anderson was tied for the NFL lead in special teams tackles before suffering a season-ending ACL tear in the Oct. 29 win against the 49ers.</p>
<p>Meanwhile, the Texans with a quarterback on a rookie deal, have veterans such as nine-year linebacker Neville Hewitt and fifth-year cornerback Ka&#8217;dar Hollman who are in the NFL&#8217;s top 20 in special teams tackles, per PFF.</p>
<p>&#8220;They don&#8217;t give up anything,&#8221; said Simmons of a kick and punt defense that is both ranked third. &#8220;What they&#8217;re taking advantage of right now is they have not had a quarterback there in some time. And so they can pay the lower half of the roster higher than what the rest of the league can because they don&#8217;t have to pay the quarterback yet. They have a bunch of good veteran special teams players and they&#8217;re taking advantage of that.&#8221;</p>
<p><strong>STATE OF TEAMS:</strong> Despite the loss of Anderson, Simmons is bullish on his young guys.</p>
<p>Robbins, he says, has bounced back from his nightmarish 10-punt opener in the rain and been solid. He particularly points to the last punt in San Francisco with 19 seconds left and a 14-point lead.</p>
<p>&#8220;It didn&#8217;t seem big to a lot of people, but it is to me, how we handled the situation at the end … We ran the play clock down. We called timeout. We talked about the rush. But then I told him, this punt has to be out of bounds,&#8221; Simmons said. &#8220;Because there&#8217;s going to come a time it&#8217;s a seven-point game, or it&#8217;s a three-point game with 20 seconds left where that ball&#8217;s got to be out of bounds. And he executed it perfectly, kicked it out of bounds.&#8221;</p>
<p>Translation: If the punt in the AFC title game with 30 seconds left had gone out of bounds, the Bengals could have been in the Super Bowl again. At the very least, overtime.</p>
<p>Simmons says one guy can&#8217;t replace Anderson because of his unique size and speed, but keep an eye on Battle. Battle, the third-round safety from Alabama, bowled over Simmons with his 18-minute interview at the NFL scouting combine that had Simmons drawing in his notes they had to have him. The only other guy he did that with this year is future Lions running back <strong>Jahmyr</strong> Gibbs<strong>.</strong></p>
<p>&#8220;Jordan&#8217;s in a bit more of a position where he can affect the game better or, or more. He&#8217;s in critical spots. He&#8217;s playing the old spots that Mike Thomas played,&#8221; Simmons said of his old captain still around on the practice squad.</p>
<p>&#8220;He&#8217;s an inside player on kickoff. He&#8217;s not had to change with the injury to Tyson … He&#8217;s trying to handle a lot right now in addition he&#8217;s got a role on defense, too. So there&#8217;s a lot going on for him. And I think he&#8217;s managed it pretty well for the most part. He&#8217;s a smart kid that you can tell has played a lot of football in his career and some things come naturally to him. Some things take work, but he&#8217;s willing to put the work in and do it.&#8221;</p>
<p><strong>SLANTS AND SCREENS:</strong> The Bengals could use a fellow Buckeye to get Texans quarterback C.J. Stroud out of the pocket, but left end Sam Hubbard (ankle) didn&#8217;t work for the second straight day …</p>
<p>You know that false start called on Orlando Brown that turned Evan McPherson&#8217;s 55-yard field goal into a punt on Sunday night? Simmons is still shaking his head.</p>
<p>&#8220;They had a player right in front of Orlando that simulated the snap count. He yelled out,&#8217; set.&#8217; You can see his head move and see him do it to Orlando,&#8221; Simmons said. &#8220;I mean, the official that called the penalty, there&#8217;s no chance he saw Orlando move. Zero. Okay. I can barely see him move on video. Just look at the (video).&#8221; …</p>

<p>SAN FRANCISCO — After three and a half years removed from the start of the pandemic, some San Francisco small businesses said they&#8217;re still struggling to get back to pre-pandemic levels.</p>
<p>No one follows the issue closer than Maneet Sohal. His livelihood depends on tourism since he owns San Francisco Deluxe Sightseeing Tours, the company that operates the &#8220;hop-on, hop-off&#8221; buses.</p>
<p>&#8220;We&#8217;re picking (the tourists) up; we&#8217;re dropping them off. Every 25 minutes, we have a new bus come around,&#8221; said Sohal. &#8220;Business is on par with last year, so it&#8217;s almost identical to what we did last year. But we&#8217;re about 70 percent of what we did in 2019 right now.&#8221;</p>
<p>He said Union Square and Downtown have been very slow, even for this Labor Day weekend.</p>
<p>Nearby businesses agreed. Pinecrest Diner near Union Square no longer opens 24/7. Manager Morena Hernandez said they had to cut back on their hours because of declining foot traffic.</p>
<p>&#8220;We&#8217;re surviving. We&#8217;re not thriving,&#8221; said Hernandez.  &#8220;I wish the city would do something with homelessness because that really affects a lot.&#8221;</p>
<p>Many businesses said the downturn is the result of a triple whammy: homelessness, crime and the pandemic, which they said only accelerated the problem. They blamed years of bad city policies that led to the worsening street conditions.</p>
<p>&#8220;I grew up here in North Beach, and it&#8217;s a beautiful city that I love. And it doesn&#8217;t seem as beautiful anymore,&#8221; said Mac Leibert, whose family owns Pier 23 Cafe.&#8221;I&#8217;d like to see the politicians get it back to the jewel that it can be.&#8221;</p>
<p>On top of crime and homelessness, a man visiting for the first time from Las Vegas said the city has a reputation problem.</p>
<p>&#8220;That&#8217;s what took me so long to come here. My wife was like, &#8216;Let&#8217;s go to San Francisco.&#8217;  And I was like, &#8216;Well, they say it&#8217;s horrible there now.&#8217; It is beautiful, the weather is great,&#8221; said Marcel, who declined to provide his last name.  </p>
<p>Marcel said his wife loved San Francisco so much that she suggested moving to the city. Marcel is a business owner and a developer who created the &#8216;Nu Media Tech Solo&#8217; social media app.</p>
<p>Despite the economic challenges closer to Downtown San Francisco, Pier 39 saw big crowds this weekend. Pier 39 spokeswoman Sue Muzzin said sales for the pier tenants are about the same this Summer compared to 2019.</p>
<p>Sohal also noticed his buses are picking up and dropping off a lot more visitors over at Fisherman&#8217;s Wharf and Pier 39.</p>
<p>&#8220;I think tourists preferred to stay on that side of town. Maybe safety is the issue,&#8221; said Sohal.  </p>
<p>He said city leaders have to do more so everyone not only survives but thrives.</p>
<p>&#8220;We&#8217;re hoping that tourism does bounce back and is even better than 2019,&#8221; said Sohal.</p>
<p>Businesses told KPIX 5, that while domestic visitors have returned, international travelers are still slow to come back to the city.</p>

<p>With the spread of COVID-19, cities across the country were forced to see the homelessness crisis in a new light: Large groups of people without a residence who slept in camps or shelters were at high risk for the transmission of COVID-19 .  In response, San Francisco opened 25 shelter-in-place hotels in just six months to protect more than 2,500 people.</p>
<p>For many residents, the hotels have become a place of refuge to recover from the physical and mental stresses of an unhindered life.  A resident&#8217;s experience of dealing with diabetes shows this crucial relationship between living and healing:</p>
<p>&#8220;[Before the hotel] I looked really, really bad.  My kidneys were shut down, my health was deteriorating rapidly &#8230; Due to a &#8230; lack of time and privacy, I didn&#8217;t pay as much attention as I should &#8230; When you are with people all the time, thoughts are difficult having to think about the pros and cons or making decisions that are so important to my health &#8230; When I moved, I was given time, medicine and support from the medical staff &#8230;[Now]I am on my diabetes.  My [blood sugar] has come down amazingly &#8230; Things are looking good.  &#8221; </p>
<p>In the hotels, all residents are examined for their health and social needs and can connect with basic care, mental health care, substance treatment, physiotherapy and social services.  Often these services are provided directly in hotel rooms.</p>
<p>Naomi Schoenfeld is a nurse and medical anthropologist who provides medical care in the hotels and is currently studying how the hotels have affected the health of residents.  She shared her experiences with us: &#8220;</p>
<p>I have seen remarkable improvements in the health of some patients.  The hotels have been very successful in connecting patients to the services they need.  People with severe leg ulcers, ulcers that just haven&#8217;t gotten better due to the instability of homelessness, are finally getting the wound care they need.  Beyond healthcare, however, it is also the simple dignity of living that has enabled patients to heal.  Being able to rest, go to the bathroom without having to walk from place to place, have three nutritious meals a day &#8211; these are the basic dignities that are essential for managing a state of health.  &#8221; </p>
<p>The shelter-in-place hotels have shown us that a radical restructuring of our housing system is not only feasible, but also curable.  Now we should build on this moment in order to usefully address the inequality of housing conditions in the city beyond the pandemic.</p>
<p>In the medical community, we know that homelessness leads to negative health outcomes and that the most effective solution to homelessness is permanent supportive housing: housing that, like shelter-in-place hotels, provides medical and social services, but not housing &#8211; In-place hotels are permanent.</p>
<p>Right now, we in San Francisco have a rare opportunity to reduce homelessness by investing in permanent supportive housing.  Between newly released Proposition C funds, state and federal emergency funds, bond money, and a declining housing market with hotels and student residences for sale, we&#8217;re in a unique position where funding and buildings are exceptionally no barrier.  It is entirely possible that the city is urgently buying at least 600 new permanent support housing units for unhodged people, including those who have not been offered hotel rooms.</p>
<p>As clinicians, we are often at the receiving end of the failures of a revolving door care system and limit ourselves to plastering solutions.  But at this moment we have the perfect confluence of opportunities to address the source of the disease.  Housing is the cure.  We ask the city to act now and purchase hotels and student housing as permanent supportive housing.</p>
